Occupational Therapist, Research, Per Diem
OverviewThe VNSNY Center for Home Care Policy & Research is seeking to hire occupational therapists to cover Brooklyn, Queens, Manhattan and/or the Bronx for a study evaluating the effectiveness of a multi-component homebased intervention to enhance physical function of patients who have ADL difficulties following a recent hospitalization.
The intervention is targeted toward VNSNY home care patients as part of a collaborative with Johns Hopkins University and will implement the Community Aging in Place - Advancing Better Living for Elders program (the CAPABLE study).
The Center is looking for an Occupational Therapist (OT) who will work with a multidisciplinary team on a grant funded intervention study.
The OT will be work closely with a registered nurse (RN) and handyman service on the study team to implement a 5-month intervention for participants who have agreed to be in the study.
The OT will visit each study intervention patient approximately 6 times over a 5-month period.
The OT will assess the study participants' environmental home safety risks using CAPABLE study specific assessment tools and will determine whether or not this has an impact on the patient's daily function.
During this assessment, the OT and the participant identify and prioritize health goals, and make plans to achieve those goals.
The OT will work with the study RN to help the participant work on the goals identified throughout the 5-month intervention.
In each session, the OT reinforces strategy use, reviews problem-solving, refines strategies (such as exercises or pain management), and provides education and resources to address future needs.
ResponsibilitiesOT assesses the home for safety (e.
g.
, holes in walkways, uneven carpeting, and absent railings or banisters).
Based on the environmental assessment, observation of ADL activities, and identification of the participant's goals, the OT and participant discuss possible environmental modifications.
OT plans specific objectives, goals and actions designed to meet client needs identified in the assessment.
Focuses on action-oriented, time-specific and cost effective plans.
OT engages the participant in problem-solving to identify behavioral and environmental contributors to performance difficulties and strategies for attaining functional goals.
OT will be responsible for ordering the supplies and home repairs/modifications with the study handyman vendor.
Participate in monthly meetings with the site Project Manager to ensure smooth communication, supervision, and adherence to intervention fidelity.
Works one-on-one with study RN to ensure adherence to the study treatment plan.
Engage patient, family, caregivers, and healthcare providers to assure that a well-coordinated treatment plan is established.
Implements the intervention study protocol as established.
Document clinical findings in electronic health record system.
Works closely with the Project Manager and other members of the study team on monitoring and tracking patient contacts and progress.
Requests necessary referrals/consultations.
Provides care in one or more care settings:
travels to patients' homes and/or other facilities with varying environments (e.
g.
, elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.
) using approved transportation options.

Bachelor's or Master's degree in Occupational Therapy from a program approved by the New York Department of Education - OR - Graduate Certificate in Occupational Therapy from a college or university whose program is registered by the State education department or accredited by a national accreditation agency required.
Valid driver's license may be required, as determined by operational/regional needs.
Experience:
Minimum of two years of experience as an Occupational Therapist required.
Clinical experience in long-term care, home health care or acute care preferred.
Bilingual (English/Spanish) verbal and reading fluency, preferredAvailable for at least 12 monthsAvailable to cover patients in all areas of assigned borough(s)Available in the daytime at least 1.
